Program: Family Foster Care or Treatment Family Foster Care
Promote permanency for children and youth in foster care by facilitating family participation and providing an independent perspective during service plan reviews. Support and promote strengths-based casework through the Family Team Conference model.
RESPONSIBILITIES:
- Convene and facilitate family team conferences, including comprehensive and reassessment planning, service planning conferences, goal changes, placement preservation/changes, and step-ups to therapeutic care.
- Submit Family Team Conference requests to ACS and coordinate with program staff as needed.
- Adhere to Family Team Conference schedules and communicate effectively with ACS and program staff.
- Promote and monitor permanency planning during all FTCs in accordance with federally mandated timeframes.
- Attend ACS transitional conferences and create/update genograms.
- Prepare and distribute monthly reports on permanency FTCs, SPRs, and transitional conferences.
- Participate in making quality decisions regarding child safety, well-being, and permanency; provide recommendations to case planners, supervisors, and other service providers, including referrals and resources.
- Collaborate with community representatives to ensure participation in conferences.
- Complete administrative tasks, including documentation and data entry of family team conferences in CONNECTIONS, meeting required deadlines.
- Facilitate up to three conferences per day.
- Ensure compliance with agency policies, procedures, and governmental regulations, including HIPAA.
- Perform other duties as assigned by supervisor.
QUALIFICATIONS:
Education/Degree:
- Master of Social Work plus 2 years of foster care experience OR Bachelor's degree plus 5 years of foster care experience.
- Exceptional mediation skills for effective group conference facilitation.
- Ability to clearly summarize solutions and facilitate consensus.
- Flexible work schedule, including evenings and weekends.
- Willingness to travel within the NYC region.
- Ability to interpret policies and procedures.
- Strong organizational, interpersonal, verbal, and written communication skills.
- Proficiency in MS Office (Excel, Word) and CONNECTIONS.
Other skills and requirements:
- Proficiency in Microsoft Word, Excel, and CONNECTIONS.
- Willingness to travel within boroughs and Long Island to various sites as needed.
- Commitment to SCO Family of Services' mission and programs.
- Ability to work effectively with diverse populations, including children and families with histories of trauma, mental health issues, or cultural differences.
- Comfort working with individuals exploring or disclosing their sexual orientation or gender identity.
- Flexibility to work beyond scheduled hours if necessary.
- Empathy and passion for child welfare.
Location of Position: Brooklyn (travel to Queens or Deer Park as needed)
