As a Family Support Specialist, you will provide essential therapeutic services utilizing the Family Centered Treatment model.

  • Implement Family Centered Treatment (FCT) Services according to model fidelity requirements.
  • Obtain FCT Certification within one year of employment to continue providing FCT services.
  • Complete necessary FCT Checkoffs and internal training within assigned deadlines.
  • Deliver FCT services primarily in clients' living environments, focusing on family dynamics.
  • Provide 24/7 on-call support to a caseload of families, requiring flexibility in your schedule.
  • Complete all Clinical Records and FCT documentation with children and families, ensuring timely entry in the Electronic Health Record.
  • Develop and maintain Service Plans in coordination with the Treatment Team, ensuring collaborative communication.
  • Conduct ongoing revisions and implement Service Plans effectively.
  • Provide psychoeducation and assist families in establishing and reaching weekly goals.
  • Make service referrals and coordinate discharge plans that meet aftercare needs.
  • Engage with community resources and support providers to facilitate effective planning.
  • Document consumer progress and participate in Quality Assurance activities.

Qualifications:

  • Bachelor's Degree in human services or related field.
  • Two years of experience working with youth or families.
  • Current Family Centered Treatment Certification is preferred.
  • State-specific qualifications may include necessary clinical registrations based on your work location.
