Assabet Valley Collaborative (AVC) is an educational service agency in Metrowest/Central Massachusetts. Our services include school programs, wraparound services, transportation services, professional development, cooperative purchasing and consulting. What is the Family Success Partnership? (FSP): FSP is a wraparound program provided to families through contracts with their local public school districts. FSP partners with schools and works closely with families to foster the connection between home and school. We assist with making community based referrals for a wide spectrum of needs, and provide case management, progress monitoring, and direct clinical intervention for families. We also provide consultation and professional development to member school districts of AVC. Our work is centered around the belief that overall student and family wellness is an integral component of a child's success in school. QUALIFICATIONS: Advanced degree in counseling field (Social Work, Counseling, Psychology), or 10+ years in human services required. Bilingual fluency in Spanish and English. Massachusetts clinical licensure desired - LMHC, LCSW, LICSW, and/or DESE School Social Worker/Adjustment Counselor. Because of the mobile, community-based nature of wraparound work, possession of a valid driver's license and a vehicle are required. Work Year: School year August - June: 2 days per week (14 hours) for 10 months Reporting hours: 14 hours per week Additional Opportunities:
- Additional hours for over flow cases if needed/wanted
- Additional summer hours if needed/wanted
- Potential for position to increase to benefits eligibility or full-time status as FSP grows
The ideal candidate would possess or demonstrate the following:

Job Description:
- Clinical case management and care coordination of approximately 15 cases over the academic school year
- Assist in navigating complex state agencies such as Masshealth, DTA, DCF, DMH or DDS
- Completing applications for a spectrum of services, including basic needs
- Accompany families to various meetings/appointments that require clinical expertise and/or psychoeducation as needed
- Organize and match care across providers and child serving systems to empower the family to meet the students needs
- Respond to the needs of our districts and stakeholders
- Provide interim clinical intervention (family, individual & group).
- Provide clinical consultation/instruction to building based teams
- Uphold discipline ethics and/or licensure requirements
- Proficient with Google platform
- Maintain caseload files with appropriate documentation and program assessments
- Travel within the MetroWest Region
- Actively embed self within assigned districts including networking, relationship building, coalition building, system of care meetings and district wide initiatives.
- Support FSP workshop development, and participate in the creation of and facilitation of Professional Development as required.
