Provide comprehensive care to patients of all ages, including physical exams, treatment, and preventive health education. Collaborate with physicians and staff, manage acute and chronic conditions, and use eClinicalWorks to support continuity of care. Ideal for experienced NPs passionate about inclusive, patient-centered family medicine.Responsibilities as the Family Nurse Practitioner (Primary Care):
- Patient Evaluation & Preventive Care: Perform comprehensive physical exams, assess psychosocial factors, and conduct screenings such as Pap smears, GC cultures, and chlamydia tests.
- Diagnosis & Treatment Planning: Evaluate clinical findings, prescribe medications and treatments, and make referrals in accordance with agency protocols.
- Primary Care Management: Provide ongoing care for acute and chronic conditions, developing holistic, risk-specific health plans for diverse populations.
- Collaboration & Coordination: Consult with physicians on complex cases, coordinate care with specialists and social services, and ensure appropriate follow-up.
- Patient Education & Compliance: Educate patients on family planning, HIV/STD prevention, and nutrition while adhering to HIPAA, agency guidelines, and clinical standards.
Qualifications for the Family Nurse Practitioner (Primary Care):
- Education: Master s degree in Nursing and graduation from an accredited Nurse Practitioner program required.
- Licensure: Current and unrestricted Nurse Practitioner license in the State of New York.
- Certification: National board certification as a Family Nurse Practitioner (FNP) required.
- Experience: 3+ years of experience providing primary care as a Nurse Practitioner to patients of all ages in a family practice or outpatient clinic setting.
- Technical Skills: Proficient in using eClinicalWorks (EHR) and other clinical documentation platforms.
- Skills & Attributes: Strong communicator with a patient-first mindset, cultural sensitivity, and a commitment to education and equitable care.
