Nurse Practitioner or Physician Assistant (Primary care)Location: Salinas, CAJob Type: Full-timeShift: Monday - Friday, 8 am- 5 pmWork Location: In person.Pay Range: $130k-$160kAbout Your Work:

• Obtain history and perform physical examination
• Complete review of medicines and related instructions
• Review home safety and psychosocial needs
• Fulfill documentation needs for STAR and HCC measures
• Obtain and document all pertinent information in patient's charts and case management notes on the same day as patient is seen; include medications and end of life preferences and drivers of preventable hospital admissions.

Qualifications:

• CA Licensed Nurse Practitioner
• 1+ Experience with Primary Care
