A growing, established PACE program in Northern Indiana seeks a Board-Certified Family or Internal Medicine physician with geriatric experience to serve as Medical Director. This leadership role offers the chance to guide an interdisciplinary team and make a meaningful difference in the lives of vulnerable seniors.

• Provide clinical leadership across clinic, home, and community-based care

• Supervise and mentor Primary Care Providers and APPs

• Lead Quality Assurance & Performance Improvement (QAPI) initiatives

• Develop medical policies, protocols, and care standards

• Participate in care plan meetings, intake evaluations, and family conferences

• Perform new and follow-up physical exams and assessments

• Represent the program at community and partner meetings

• Shared provider on-call responsibilities

BC in Family or Internal Medicine Required BC or CAQ in Geriatric Medicine Highly Preferred Indiana medical license or eligibility Required DEA, CDS, and BLS Required by start At least 1 year working with frail or elderly populations Required Prior supervisory/leadership experience Required Proficient in Microsoft Office & EMR systems Valid driver's license, vehicle, and insurance
