Key Responsibilities:

• Provide primary care services including routine check-ups, preventative care, and chronic disease management.
• Diagnose and treat a wide range of acute and chronic medical conditions in patients of all ages.
• Order and interpret diagnostic tests, lab work, and imaging as necessary.
• Coordinate care with specialists, behavioral health, and ancillary services as needed.
• Maintain accurate and thorough documentation in the electronic medical record (EMR).
• Educate patients and families about health maintenance, disease prevention, and treatment plans.
• Participate in quality improvement initiatives and uphold best practices in patient care.

Qualifications:

• Medical Degree (MD or DO) from an accredited institution.
• Board Certified or Board Eligible in Family Medicine.
• Licensed to practice in the state of New York (or ability to obtain).
• Current DEA registration.
• Excellent interpersonal and communication skills.
• Strong commitment to providing high-quality, culturally sensitive care.

Preferred Experience:

• Experience working in an urban, community-based outpatient setting.
• Multilingual abilities (Spanish, Russian, Mandarin, or others) are a strong plus.
• Experience with value-based care models and EMR systems (e.g., eClinicalWorks, Epic).
