You'll lead with empathy, precision, and cultural sensitivity to help patients not just get better, but thrive. Comprehensive Primary Care Manage a dedicated panel of adult patients with a wide spectrum of acute and chronic conditions Conduct initial evaluations for mental health concerns (e.g., anxiety, depression) with compassion and skill Order and interpret diagnostic tests, using evidence-based medicine (EBM) and high-value care principles Prescribe medications and therapeutic interventions with confidence and within scope of practice Counsel patients on treatment plans, preventive screenings, and lifestyle improvements Health Advocacy & Outreach Be a voice for wellness-lead health promotion efforts and community outreach initiatives Help build trust and drive engagement at the site and in the surrounding community Team Collaboration & Quality Care Join forces with a dedicated, interdisciplinary team to deliver seamless, coordinated care Participate in team huddles, pre-visit planning, and clinic-wide meetings Contribute to continuous quality improvement projects, peer review, and evidence-based innovation Documentation & Compliance Keep accurate and timely records in the EMR system, including visit notes, billing, and reports Stay current on licensure, certifications, and continuing education Follow all clinical, legal, and ethical standards-including HIP nd mandated reporting Qualifications: Certified Family Nurse Practitioner (FNP) or Adult Nurse Practitioner (ANP) Active NYS license and current DEA certification BLS certification and infection control training (current) Exceptional bedside manner, communication skills, and cultural competency A heart for service and a passion for health equity Salary : $140000 - $160000 Please Email: [email protected] #INDALL#LI-JZ125-37491
