CenterWell Senior Primary Care Centers is seeking a dedicated Family, Internal, or Geriatric Medicine Physician to join our team! You have the opportunity take a quality focused, and hands-on approach, addressing patient needs inside the exam room and look beyond to understand how social determinants of health are impacting health outcomes. We have a unique care model that sets us apart for patients and providers. We are experts in senior-focused primary care with a multidisciplinary approach. Key Responsibilities: * Work Schedule: Monday-Friday, 8:00 AM - 5:00 PM; no nights, no weekends, no holidays! * Dedicated administrative time weekly * Patient Care: * Lower patient volume with focus on quality and relationships * Longer appointment times = more 1:1 time with patients * Serve as a lead medical expert within the Care Team, actively participating in daily huddles. Required Qualifications: * Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO). * Board-certified or board-eligible in Geriatric Medicine, Family Medicine, or Internal Medicine. * Valid, unrestricted medical license in the state of desired employment (or willingness to obtain). * Current, unrestricted DEA license. Preferred Qualifications: * Experience managing a Medicare Advantage patient panel with a solid understanding of best practices in a coordinated care environment. * Knowledge of Medicare guidelines and coverage. * Familiarity with HEDIS quality indicators.
