Description

As a Family Finding Coordinator for foster care, you will identify and engage family and non-relative extended family members for children and youth in foster care using Kevin Campbell's Family Finding Model. You will be a liaison between the youth's diverse teams, Department of Social Services and identified natural supports to create a strong network of support for youth to expand on the idea of what permanency can mean for a child in care. You will report to the Family Finding Supervisor. This is a hybrid position with community travel required.

Responsibilities:

  • Build comprehensive genograms through search engines, case mining and by speaking to the youth, families, and LDSS case workers

  • Engage with the youth to discuss important people in their lives i.e. family and family support

  • Participate in Service Plan Review meetings or other related permanency meetings

  • Engage identified fictive and nonfictive kin to assess their willingness and capability of providing support to youth in care

  • Identify and document at least 25 family/kin for each child through diligent search activities including the internet, social media and databases, case record, and interviews.

  • Facilitate communication between the youth's identified support system with the treatment team and LDSS to engage them in the youth's permanency process

Requirements and Education:

  • Bachelor's degree required

  • 1+ years of child welfare experience and comfort handling confidential information required

  • 1+ years of experience in Microsoft Office Suite and other relevant software applications required

  • Valid NYS driver's license required
