FAMILY COUNSELOR II, STEPPING STONES-FULL TIME - Meadville Medical Center : Job Details

JOB DUTIES Maintains communication with other members of the chemical dependency treatment team. Acts as Case Manager for patients, functioning as a social worker when necessary to prepare patients for discharge. Makes appropriate, legible, entries in patient record Counsels patients on an individual basis. Conducts group therapy as primary facilitator. Prepares Master Treatment Plans, Psychosocial interview forms and meaningful Aftercare Plans in concert with the needs of the patient. Works variable hours as directed by the Program Supervisor to meet department and patient needs. Conducts family education sessions and works on an individual basis with families when so directed. Functions as a member of the Treatment Team. Possess knowledge of and utilizes referral sources and other agencies necessary to provide therapeutic support for the patient. Collaborates with other hospital departments in order to maximize the effect of the current hospital admission.
