Summary Job Description: The services coordinator is responsible for developing the Individualized Service Plan (ISP) implementing and maintaining the ISP. A person-centered approach is integral and essential in developing a meaningful ISP that captures the individuals valued outcomes and personal goals. The ISP must include the expectations, preferences, interests, talents, skills, desires, needs, and relationships of the individual. The information must present a balanced view of the individual and include such areas as family, personal life, physical and mental health, social life, education, recreation, vocational or job interests and spiritual interests. Rate: $30 THIS ROLE IS TO ACT AS BRIDGE BETWEEN PATIENT THAT AGE OUT OF SCHOOL PROGRAMS AND ARE PLACED IN FOSTER HOMES. CANDIDATE IS THE BRIDGE BETWEEN FOSTER HOMES AND PROGRAM MANAGERS IN CASE ANY CHANGES NEED TO BE MADE. Family Care Service Coordinator can work out of the Wading River location and will have some local travel to visit patients in homes but must have a valid drivers license as they will also oversee a caseload in Brooklyn that they will occasionally have to go to. SCHEDULE TBD: Flexible but Mon-Fri 9am-5pm Principle Responsibilities: • Advocate for each enrolled individual who requires or chooses to have an advocate • Ensures timely completion of all necessary waiver enrollment processes and annual redetermination of the need for level of care in an ICF/MR. • Listens to the individual and advocate to understand what is important to the individual. • Assists with planning and deciding which services or supports are priority. • Design and completes (within 60 days of the individual moving into Family Care) the individual's approved ISP and gives copy to the individual, advocate, family care provider, and other service providers.
