Overview

Responsible for developing, cultivating, and maintaining relationships with families within the Health Families Program through home visitation.

Responsibilities
  • Establish and maintain positive relationships: Interact professionally, courteously, and appropriately with patients, visitors, and other employees. Behave in a manner that maintains and enhances the positive public perception of BronxCare Health System and its staff.
  • Participate in quality improvement activities: Contribute to data collection, analysis, implementation, and compliance with risk management and claims activities. Support and participate in Continuous Quality Improvement (CQI) teams, adhering to BronxCare Health System policies on safety, security, risk management, infection control, and customer service.
  • Family assessment and goal setting: Conduct assessments to determine family needs and develop goals collaboratively with the family.
  • Home visits: Make visits to families' homes as per the level and frequency required by HFA standards, demonstrating flexibility.
  • Support during agency and medical visits: Accompany families to appointments when necessary.
  • Develop family goals and outcomes: Assess families and develop service plans based on their needs and input.
  • Documentation and reporting: Track services and generate written reports related to service delivery, case records, and data collection instruments.
  • Additional duties: Assist in special projects and perform other duties as delegated by the Program Manager and Supervisor.
  • Training and travel: Attend all required Healthy Families training and be prepared to travel within and outside the state as needed.
  • Qualifications
    • At least one (1) year of experience working in a child care facility such as a Day Care Center or Head Start Program.
    • Appropriate licensure based on degree, e.g., MSW or CSW.
    • Certificate in Child Care.
    • Associates degree in Psychology, Social Work, or Elementary Education.
    • Computer literacy.
