Job Title: Facilitator

Division/Program: Teammates Mental Health - Santa Clara

Starting Compensation: 28.68 - 34.41 USD Per Hour

Working Location: San Jose: North San Jose Center (In-Person Position)

Working Hours/Shift: Monday-Friday, AM shift

What you bring to SBHG (qualifications):

• Facilitator I: Associate Degree in Psychology, Social Work, Rehabilitation Therapy, Nursing or equivalent.
• Facilitator II: Bachelor's degree in Psychology, Social Work, Rehabilitation Therapy, Nursing or equivalent.
• Facilitator III: Master's Degree in Psychology, Social Work, Rehabilitation Therapy, Nursing or equivalent.
• Four (4) years direct service experience providing direct in-home or community-based services.
• Valid California Driver's License.

How you will make a difference (job overview):

This position has primary responsibility for providing wraparound, mental health, and case management services for the TEAMMATES Program. Services may include engagement, assessment, Child and Family Team formation and implementation, development of the Plan of Care and budget, and case management services to adolescents and their families referred to the TEAMMATES Program. The Facilitator convenes the Child and Family Team with the Child and Family Specialist and Parent Partner and requests and coordinates the delivery of wraparound services from TEAMMATES staff. He/She documents services, bills for mental health services, and completes tracking and outcomes information and reports.

Division/Program Overview:

• Provides mental health services to youth involved in juvenile and child welfare systems.
• Participates in 24/7 on-call system; pay differential.
• Wraparound approach provides services to entire family.
• Services provided in community, home-based, and juvenile justice systems (Ranch, juvenile hall).
