Position Overview We are seeking an experienced External Care Manager to join our team in Monmouth County, NJ . This role is ideal for someone with a strong background as a Clinical Liaison , exceptional relationship-building skills, and a passion for guiding patients and families through the admissions process. A proven track record in healthcare admissions is highly valued. Key Responsibilities Serve as the primary point of contact for hospitals, physicians, discharge planners, and other referral sources within Monmouth County . Conduct assessments to determine appropriate levels of care and facilitate smooth transitions into our facilities. Build and maintain strong referral relationships to promote Complete Care's services. Coordinate with internal admissions teams to ensure timely and efficient patient placement. Provide education to patients, families, and healthcare partners regarding services, programs, and insurance coverage. Represent Complete Care at community events, health fairs, and networking functions to enhance visibility and outreach. Qualifications Clinical Liaison or related healthcare outreach experience required. Strong understanding of post-acute care, skilled nursing, and rehabilitation services. Admissions experience within a skilled nursing facility or hospital setting is a strong plus. Exceptional communication, interpersonal, and organizational skills.
