Become the Case Manager you've always wanted to be
- Collaborate with an interdisciplinary team to guide treatment plans based on patient needs and preferences.
- Coordinate with team members to create tentative discharge plans and backup strategies.
- Engage in planning and executing successful patient discharge experiences.
- Monitor patient experiences across quality, timeliness, service appropriateness, and expectations.
- Lead weekly team conferences and manage treatment plan adjustments.
- Complete necessary case management documentation and reports.
- Stay informed on regulations, company policies, and departmental operations.
- Analyze case management reports to enhance care delivery.
- Participate in Acute Care Transfer meetings to identify trends and improve outcomes.
- Meet patients and families as per the Patient Arrival and Initial Visit Standard within 24 hours of admission.
- Conduct assessments and document case management addendums within 48 hours of admission.
- Educate patients and families about rehabilitation and the role of the Case Manager.
- Schedule and conduct family conferences as required.
- Assist patients in securing necessary resources to prevent discharge delays.
- Ensure compliance with regulations in orthotics and prosthetics ordering and payments.
- Make timely referrals and ensure proper documentation for post-discharge providers.
- Verify discharge and payer-related information for accuracy in the patient record.
- Engage in the utilization review process, including data collection and trend analysis.
- Partake in case management on-call schedules as needed.
Qualifications
License or Certification:
- Must be qualified to independently conduct assessments within the scope of practice of your discipline (e.g., RN, SW, OT, PT, ST, Rehabilitation Counseling).
- If state licensure is required for your discipline, you must hold an active license.
- Must meet eligibility for CCM® or ACM certification upon entering this role or within two years of being in the position.
- CCM® or ACM certification required or must be obtained within two years if promoted to Case Manager II.
Minimum Qualifications:
- For Nursing: Must possess a minimum of an Associate Degree in Nursing, with RN licensure (BSN preferred). A diploma is acceptable only in states where this is the minimum licensure requirement.
- For other eligible licensed or certified healthcare professionals: Must possess a bachelor's degree, with a graduate degree preferred.
- 2 years of rehabilitation experience is preferred.
