Role Overview:

The Entry-Level Care Coordinator plays a vital role in supporting low-risk Enrollees in attaining their best health outcomes. With relevant experience and education, you will work both over the phone and in person with Enrollees to complete vital assessments and maintain their personalized care plans.

Key Responsibilities:

• Provide care coordination services for low-risk members.
• Act as the primary point of contact for inquiries and assistance.
• Conduct surveys and assessments with Enrollees and/or their caregivers.
• Perform periodic field assessments as needed.
• Develop and update Personalized Care Plans.
• Conduct medication reviews.
• Monitor health, functional, and mental status over time.
• Facilitate services in the least restrictive environments and assist with transitions between care settings.
• Connect Enrollees to community resources that promote independent living while delaying potential nursing facility placement.
• Coordinate with social service providers, including local health departments, social services, and community organizations.
• Refer Enrollees to additional resources as appropriate.

Education:

• Bachelor's Degree in a health or social services related field.
• Preferred: Certification as a Case Manager or Community Health Worker.

Licensure:

• Valid driver's license with proof of car insurance.

Experience:

• 1 to 3 years of experience in social services or care management/coordination, ideally working with clients in home or community settings, including physical health, behavioral health, and social services.
• Preferred: 3 to 5 years of experience in Case Management.
• Preferred: Experience in remote or telephonic environments.
