• Enroll qualified participants from various sources up to capacity of the Program of All-Inclusive Care for the Elderly (PACE).
• Coordinate all aspects of enrollment and disenrollment.
• Liaise with appropriate health and related entities; including but not limited to hospitals, home care, and hospice as well as the broader community.
• Contribute to accurate reporting of program enrollment and relevant statistics.
• Perform communication and outreach activities.
• Plan and coordinate program events for the purpose of general education on services provided.
• Develop appropriate referral sources on an ongoing basis.

• Education: High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required
