This remote position requires you to engage with individuals in your community who are utilizing the TMG IRIS Consultant Agency. You will spend much of your time visiting IRIS participants in their homes, helping to make a meaningful difference in their lives. While routines exist, every day will bring unique experiences.

In this role, you will be responsible for completing the Adult Long-Term Care Functional Screens (LTC-FS) for participants in the Wisconsin IRIS program—a Medicaid long-term care option for older adults and individuals with disabilities. Your duties will include conducting annual rescreens and change-in-condition screens with an emphasis on ensuring program eligibility. Job Duties

  • Conduct face-to-face meetings with IRIS participants to execute the screening process.
  • Complete the Adult Long Term Care Functional Screen (LTC-FS) in accordance with the Wisconsin Adult LTC-FS Instructions.
  • Verify screen results through contact with IRIS Consultants, Medicaid Personal Care agencies, and confirm diagnosis information with physicians and the Social Security Administration as necessary.
  • Uphold exceptional documentation standards and program regulations while ensuring screens are completed in a timely manner.
  • Enhance screening skills through participation in weekly team meetings, monthly All Screener Meetings, and ongoing trainings.
  • Perform additional duties as assigned by management.

Job Qualifications

REQUIRED EDUCATION:

Bachelor's Degree in a health, human services, or related field (e.g., social work, psychology) or graduation from an Accredited School of Nursing.

REQUIRED EXPERIENCE/KNOWLEDGE, SKILLS & ABILITIES:

  • At least 1 year of experience in working with target groups such as adults with physical or intellectual disabilities or older adults.
  • Excellent written and verbal communication skills with strong attention to detail.
  • Proficient in computer and software applications, including the Microsoft Office Suite.
  • Strong customer service orientation.
  • Effective organizational and time management skills.
  • Able to work collaboratively as part of a team or independently.
  • Flexibility to adapt to a dynamic work environment and changing organizational needs.
  • Minimal travel requirements (5%).

REQUIRED LICENSE, CERTIFICATION, ASSOCIATION:

  • Must successfully complete the Wisconsin Adult Long-term Care Functional Screen certification modules (achieving at least 80% on each) within the first week of hire and maintain certification.
  • If a graduate from an Accredited School of Nursing, must possess an active, unrestricted Wisconsin Registered Nursing (RN) license in good standing.
  • A valid driver's license, adequate auto insurance for job-related travel, and the ability to travel throughout Wisconsin are necessary.

PREFERRED LICENSE, CERTIFICATION, ASSOCIATION:

Currently or previously certified in the Long-Term Care Functional Screen.
