Position Summary: The Discharge Planner provides discharge planning activities for patients admitted to acute and residential services. This position helps complete discharge planning reflective of a basic understanding of the needs of people diagnosed with a mental disorder.
QualificationsExperience/Education: Graduate of an accredited college in a social work program or related field. A minimum of three years of experience in the care of mental health patients preferred. Must possess CPR (AED) certification and successful completion of CPI training including appropriate use of seclusion and restraint prior to first shift.
Skills: Knowledge of external review organization, payor sources, financial planning and contracting (ie. Managed care, Medicare and Medicaid, Tricare) preferred. Must have required 20 hours of continuing education, CPR, First Aid, and CPI as provided by the facility within 90 days of employment.
