Isaacs Neighborhood Center (Isaacs Center) are well-established Manhattan community-based agencies that support services to approximately 30,000 New Yorkers. We are two of New York City's leading human services organizations meeting the needs of children, youth and families, homeless individuals, and older adults through-out Manhattan and New York City. The Isaacs Center focuses primarily on the needs of children and low-income families, out-of-school and out-of-work youth, and aging New Yorkers. We operate at community centers located in the neighborhoods of East Harlem and Yorkville, and seek to deliver programs that are impactful, innovative, and intergenerational. Position Summary:The Director of Social Services is responsible for planning, implementing, and overseeing programs and services that support the emotional, social, and psychosocial well-being of residents in a senior housing community. This individual serves as an advocate for residents and their families while ensuring compliance with all relevant regulations and standards. The ideal candidate brings empathy, leadership, and expertise in aging services and case management. Schedule: 35 hours per week Salary Range: $83,500 to $90,000 Candidates must have the required qualifications to be considered for the position. The salaryoffer will be basedonthe candidate's number of years experience in the exact orcomparableposition and additional preferred qualifications will be taken into account. Roles, Responsibilities, and Essential DutiesPrimary Duties:
  • Conduct intake and assessment upon admission and at regular intervals.
  • Provide counseling, crisis intervention, and support for residents and families.
  • Serve as liaison between residents, families, healthcare providers, and community agencies.
  • Design and oversee social services programs that promote mental and emotional well-being, social engagement, and overall quality of life.
  • Coordinate support groups, educational workshops, and family meetings.
  • Maintain accurate and timely documentation in accordance with state and federal regulations, including assessments, care plans, progress notes, and incident reports.
  • Ensure compliance with HIPAA and resident rights policies.
  • Support state inspections and audits by maintaining up-to-date records and reports.
  • Supervise and mentor social services staff, volunteers and interns.
  • Participate in interdisciplinary care planning and quality improvement initiatives.
  • Provides assistance with Benefit and Entitlement applications, recertifications, or makes appropriate referrals for additional assistance and services.
  • Develops and implements all performance measurement/management processes, including collecting, submitting, and analyzing all program data to meet strategic priorities and contractual requirements.
  • Oversee and organize internal and external staff development and training programs
  • Oversee the submission of invoices and other vendor documentation into the accounting system
  • Track and analyze programming trends to evaluate effectiveness and inform future service planning.
  • Conduct targeted outreach to clients to increase engagement, address specific needs, and promote available resources.
  • Conduct and facilitate regular wellness and safety checks to ensure the well-being of residents.
  • Perform home visits as needed to assess living conditions and provide support services.
  • Collaborate effectively with building management to address resident needs, coordinate services, and resolve safety or maintenance concerns.
Qualifications:
  • Master's degree in Social Work or related field required. Licensed (LMSW) preferred but not required.
  • Minimum 3-5 years of experience in social work, preferably in senior housing, long-term care, or related healthcare settings.
  • Experience working with older adults, dementia care, and end-of-life issues preferred
  • Strong knowledge of psychosocial aspects of aging and elder care.
  • Excellent interpersonal, communication, and conflict-resolution skills.
  • Strong organizational and documentation abilities.
  • Empathy, patience, and cultural sensitivity.
  • Proficiency in Microsoft Office (MS. Word, Excel, PowerPoint, etc.)
  • Primarily an indoor setting in a residential community.
  • Regular walking throughout the facility to interact with residents and staff.
  • Occasional evenings or weekends for meetings or emergencies.
  • Must be able to lift up to 25 lbs occasionally.
  • Bilingual, English/Spanish or English/Chinese or English/Russian highly preferred.
