Director of Nursing – Assisted Living
Compensation: $150K - $160K per year
Job Type: Full-Time, Monday–Friday, On-Site
Our client, is seeking an experienced and compassionate Director of Nursing (DON) to oversee clinical operations across multiple residential care settings. This senior leadership role is pivotal in ensuring high-quality, resident-centered care while maintaining full compliance with regulatory requirements.
Responsibilities
- Lead and supervise all nursing and clinical care services across Adult Homes (AH), Assisted Living Residences (ALR), Enriched Housing Programs (EHP), Enhanced Assisted Living (EALR), and Special Needs Assisted Living (SNALR).
- Ensure ongoing compliance with all New York State Department of Health (DOH) regulations and internal policies.
- Oversee the creation and implementation of individualized service plans (ISPs) in collaboration with care managers and administrators.
- Provide leadership, coaching, and evaluation for nursing staff, and supervise aides involved in medication management and personal care.
- Conduct initial and follow-up clinical assessments for residents, and ensure timely reassessments at intervals or as needed.
- Maintain thorough and accurate clinical documentation, data reports, and quality assurance records.
- Collaborate on policy and procedure development, review, and updates for clinical operations.
- Serve as a key point of contact for residents, families, healthcare professionals, and interdisciplinary team members.
- Provide leadership and intervention during clinical emergencies or health crises.

Requirements
- Active Registered Nurse (RN) license in the State of New York
- Bachelor's degree in Nursing (BSN) preferred; equivalent experience considered
- 5–7 years of clinical experience in assisted living, home health or long-term care settings, including 4+ years in a leadership or supervisory role
- Strong knowledge of New York State Adult Care and Assisted Living regulations
