SUMMARY OF POSITION:

The Director of Nurses (DON) is the lead member of the Senior Leadership Team overseeing policy and procedures for Nurse Practitioner practice. In this role, the DON serving as the Lead Nurse Practitioner contributes to defining the overall business strategy and direction of the organization. This position works closely with the overall clinical vision for the nurse practitioner in order to not only provide high quality medical care for patients, but also align staff to meet the changing environment in healthcare: the shift to population health management, value-based contracts for care, and risk pool management. The position provides program and policy oversight, expertise and leadership to ensure the delivery of affordable, quality healthcare services. In addition, the Lead Nurse Practitioner provides clinical expertise and leadership in the design and implementation of new projects and innovations for the service line. The Lead Nurse Practitioner will continue to improve performance on the existing HRSA service contract, both in terms of cost benchmarks and quality measures benchmarks. This individual must become a member of the medical staff and maintain an active practice within the Gotham Health Center. The Lead Nurse Practitioner performs related work as assigned by the office of Gotham Health Chief Medical Officer.

PRINCIPAL DUTIES AND MANAGEMENT RESPONSIBILITIES:

The Lead Nurse Practitioner duties shall include, but not limited to:

  • Building out clinical strategy as it relates to program and policy for ambulatory care transformation across Gotham Health
  • Leads and implements the clinical direction for nurse practitioners
  • Participate in management business and clinical quality strategy development and implementation
  • Builds and leverages cross functional collaborative relationships with other service lines and programs (e.g. closing gaps in care in clinical care metrics, ACO) to achieve shared goals and standardization
  • Focus teams and organization units on visions and distinctive strategies that improve short and long-term performance
  • Availability for on call (critical labs and weekend/Holiday coverage as needed)
  • Conducts history, psychosocial evaluations and physical examinations
  • Analyzes laboratory results and follows-up on abnormal values
  • Assesses and treats acute illnesses according to established standards of care
  • Follows-up on chronic illnesses
  • As a PCP, has a defined panel of patients
  • Administers first-aid when necessary and is part of a facility emergency response team
  • Formulates a plan of care management for defined panel of patients including other disciplines as required to assist patients to a desired state of wellness
  • Completes patient correspondence (i.e. forms or letters)
  • Maintain accuracy and completeness of all patient records
  • Actively participates in departmental/facility/network in-services, meetings and trainings
  • Actively participates in the Medical Board/Department Committee meetings
  • Participates in Quality Improvement activities; develop and implement special projects (e.g. Quality Assurance Studies)
  • Acts as a preceptor for health practitioners in training, including medical students, residents, NPs, and PAs
  • Provides on-going supervision and/or evaluation of Medical Office Assistants (MA)
  • Maintains patient safety and infection control guidelines; follows established infection control protocols
  • Maintains data relative to Quarterly and Yearly Reports to the NYS DOH
  • Follows corporate and hospital regulations, policies and procedures
  • Fosters good customer service in daily activities and addresses patients, parents, and school personnel in a courteous and respectful manner
  • Maintains a professional working relationship with other health care professionals
  • Operates equipment and performs job related duties in a safe manner; supervises maintenance of equipment and supplies
  • Recognizes safety hazards and initiates appropriate action to eliminate hazards and prevent reoccurrences
  • Must be knowledgeable of Gotham Health mission/vision and uphold the mission in demonstrating the commitment to provide safe, high quality care to its patients.
  • Performs other duties as assigned within the scope of practice
  • Collaborates with the office of the Chief Nurse Executive
  • Acts as a liaison with NYSNA for all nurse practitioner related issues

CLINICAL STANDARD SETTING

  • Integrate evidence-based clinical practices wherever possible, building this into new and existing patient care delivery systems
  • Provide direct patient care at a minimum of 6 clinical sessions per week

DESIGN OF CLINICAL INTERVENTIONS AND DELIVER MODELS

  • Keeps abreast of emerging models in health care delivery
  • Identify and define new and innovative strategies to achieve the goals of population health and financial sustainability

QUALITY ANALYTICS, CLINICAL REPORTING AND RISK MANAGEMENT

  • Lead clinical and population health analytics for the service line (e.g. patient-level risk stratification)
  • Develop and monitor quality measures for new payment models (value based/risk-based payment structures
  • Works closely with the Chief Nurse, Quality, Medical, and Administrative Officers to achieve effective care management processes and develop new initiatives to improve clinical quality
  • Clinical lead for survey preparation and implementation

ASSESSMENT OF CLINICAL PROVIDERS

  • Evaluate Gotham's Nurse Practitioners
  • Create frameworks for performance evaluation of NP's (e.g. peer review, professional practice evaluations, etc.)
  • Partners and collaborates with the provider staff and management team, facilitating teamwork shared goals. Help represent provider NP's viewpoints to administration and relay administrative views to nurse practitioner staff
  • Monitor nurse practitioner's work ques in EPIC

FINANCE & BUDGET:

  • Assist in developing operating budget for salary and supply areas. Monitors variance activity on a monthly basis.
  • Monitors managed care, third party reimbursement and accounts receivable management, including cash collections. Keeps current and anticipates changes in Federal and State reimbursement regulations. Consistently reviews department budget and implement cost containment strategies as needed.
  • Responsible for the region nurse practitioner's productivity.

PROVIDER RECRUITMENT STRATEGY

  • Develop and implement strategies for nurse practitioner's attraction and retention
  • Design career paths for nurse practitioner's leaders
  • Performs other related duties as requested by the Chief Medical Officer of Gotham Health System

Minimum Qualifications

1. A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and

2. Current certificate as a Nurse Practitioner issued by the NYSED; and

3. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).

Department Preferences

CERTIFICATION(S)/LICENSE(S):

  • Holds a valid controlled substance registration certificate (DEA Registration).
  • Evidence of current CPR with AED certification; (with) ACLS and PALS certification preferred but not required

EDUCATIONAL LEVEL:

  • 4-10 Years' experience as a practicing FNP
  • Graduate of an accredited school of nursing.
  • Master of Nursing from an accredited institution

EQUIPMENT/MACHINES OPERATED:

  • Office work, ability to utilize computers, printers, fax machines
  • Medical devices used in the course of caring for patients (glucometer, blood pressure machine, etc.)

COMPUTER PROGRAMS/SOFTWARE OPERATED

  • Computer-MS, Word, Power point, Excel, Publisher Data Entry.

KNOWLEDGE & SKILLS:

  • Excellent analytical, critical and abstract reasoning skills, plus excellent organization skills.
  • Excellent communication skills are essential to interface effectively with diverse internal and external professional groups and committees
  • Experience and knowledge in strategic planning, grant proposals, leadership and resource planning.
  • Knowledge of financial, medical record system, scheduling programs and PCMH required
  • High proficiency in MS Word, Advanced Excel, Power Point, One Note, Outlook
