Director of Hospice After Hours Programs - VNS Health : Job Details

The Hospice Clinical Director of After Hours and Crisis Care is responsible for overseeing the clinical operations of the after-hours teams, including triage and field; as well as Nurse Practitioners and Crisis Care, ensuring that patients receive high-quality care in accordance with hospice standards and regulations. They are responsible for the overall delivery of quality hospice service to patients and families, in congruence with rules and regulations. They act as a champion of education, training, and delivery of quality care after hours and during times of Crisis Care and are a resource that provides guidance and support to their respective teams. They are a critical part of team manager orientation and ongoing education,growth and retention. In this role, they work side-by-side with the clinical and operational team to solidify the highest quality clinical patient care. This is a hybrid role, that acts as a liaison between the day leaders and After-Hours team.
