Your Role and Responsibilities

As the Director of Case Management, you will:

• Manage the daily operations and staffing of the Case Management department.
• Oversee the interdisciplinary care plan and discharge planning processes, ensuring services meet quality indicators and patient outcomes.
• Advocate for patients and families to ensure that all needs are met.
• Provide training and support for Case Managers and staff on effective case load management and regulatory compliance.
• Develop partnerships with insurance companies, employers, and healthcare networks.
• Recognize and celebrate the achievements of your team.

Qualifications for Success

• Current CCM® or ACMTM certification preferred.
• Licensed clinicians (RN, OT, PT, ST or Rehab Counselor) are ideal.
• A bachelor's degree required; a graduate degree is preferred.
• At least three years of hospital-based Case Management experience, including expertise in Utilization Review and Discharge Planning.
• Ability to work flexible hours, including weekends and holidays, as needed.
