Your Role

As the Director of Case Management, you will:

• Manage the daily operations and human resources of the Case Management department.
• Oversee interdisciplinary care planning and discharge processes to ensure quality patient care and management.
• Act as a patient and family advocate to ensure their needs are met effectively.
• Guide and mentor Case Managers and staff, providing training on caseload management and operational standards.
• Build robust relationships with insurance providers, healthcare networks, and employers.
• Celebrate and recognize the achievements of our dedicated team.

Qualifications

• Preferred: Current CCM® or ACMTM certification.
• Must be able to conduct independent assessments within your professional scope.
• If applicable, hold an active license in the state.
• Nursing candidates should have a BSN and RN licensure.
• Other eligible professionals need a minimum of a bachelor's degree; a graduate degree is preferable.
• Required: Three years of hospital-based Case Management experience, including Utilization Review and Discharge Planning.
• Flexibility to work various shifts, including nights, weekends, and holidays.
