Your Role and Responsibilities

• Manage the daily operations and human resource needs of the Case Management department.
• Oversee interdisciplinary care plans and the discharge planning process, focusing on patient care outcomes and key care indicators.
• Act as an advocate for patients and their families, ensuring tailored service delivery.
• Provide guidance to Case Managers, including training on caseload management and compliance with regulations.
• Build and maintain relationships with insurance providers and healthcare organizations.
• Celebrate the successes of our dedicated team.

Qualifications

• Current CCM® or ACMTM certification is preferred.
• Ability to independently conduct assessments relevant to your discipline.
• Active licensure is required, if applicable.
• For Nursing: Bachelor's degree in nursing (BSN) with active RN licensure.
• For other healthcare professionals: Minimum of a bachelor's degree; a graduate degree is preferred.
• Minimum of three years of hospital-based Case Management experience, including Utilization Review and Discharge Planning.
• Flexibility to work on weekends, evenings, and holidays as needed.
