Embrace Your Aspiration as a Case Management Director

In this role, you will:

  • Oversee the day-to-day operations and human resource management of the Case Management department.
  • Manage interdisciplinary plans of care and the discharge planning process, focusing on patient care outcomes and key care indicators.
  • Advocate for patients and their families ensuring services meet their needs.
  • Provide training and support to Case Managers and staff to enhance their skills and knowledge.
  • Build strong relationships with insurance companies, self-insured employers, and other healthcare networks.
  • Celebrate the successes and contributions of our dedicated team members.

    • Qualifications

    • CCM® or ACMTM certification is preferred.
    • Must be qualified to independently conduct assessments within the scope of your discipline.
    • If applicable, hold an active licensure in the hospital's state.
    • For Nursing candidates, a bachelor's degree in nursing (BSN) with RN licensure is required.
    • Other healthcare professionals should possess at least a bachelor's degree; a graduate degree is preferred.
    • Minimum of three years of hospital-based Case Management experience, including Utilization Review and Discharge Planning.
    • Flexibility to work on weekdays, weekends, evenings, or holidays as needed.
