Director Home Health - Wesley Retirement Services : Job Details

Our Home Health team provides services to support clients and their caregivers in their homes. Our services range from care for chronic illnesses such as diabetes, cardiac issues, and kidney problems to shorter-term care following an accident or surgery. We also provide care when clients finish a stay in short-term rehabilitation at one of our communities. We are looking for a Director of Home Health to focus on outcomes, quality, and growth. Responsibilities will include: Establishes strategic direction for the department and creates and implements plans to achieve stated goals. Oversees compliance and management activities of the Home Health Department. Responsible for survey readiness and overall success. Recognizes and celebrates success at the individual and team levels.
