SummaryLeads, manages, and facilitates transformational community and systems change efforts, by leveraging data and community input, to improve the health of vulnerable populations within the Leonard Parker Pool Insitute for Health (LPPIH) team. Develops and implements innovative and collaborative strategies to address social determinants of health and vital conditions in the community. Supports internal stakeholders and collaborative external partners to understand the root causes and dynamics of social drivers that impact health in order to design and iterate strategies to address those social drivers and vital conditions within a place-based framework. Develops and manages relationships with local stakeholders across all levels of the community, including residents, community-based organizations, systems leaders, and business leaders. Develops and manages relationships with state and national partners as well as cross-sector health improvement and community development initiatives.
