The Role:

As the Director of Care Management RN, reporting directly to the Chief Nursing Officer, you will oversee care management operations at Providence Portland Medical Center and Providence Hood River Memorial Hospital. This includes management of nurses, social workers, and administrative coordinators. Your mission will be to ensure our Care Management Departments are equipped to surpass customer expectations while adhering to all legal, accreditation, and certification standards. Your leadership will provide the necessary tools—personnel, equipment, and education—to sustain our high standards of care. By partnering with staff, you will facilitate continuous systems analysis and improvement, providing visionary leadership to our ministry departments.

What You'll Do:

  • Strategic Leader: Provide leadership and strategic planning to operationalize care management and coordination services for at-risk patients, standardizing processes where applicable.
  • Compliance Champion: Uphold all principles outlined in the Commitments and Compliance Integrity Program, ensuring adherence to Medicare and Medicaid regulations.
  • Patient Advocate: Respect Patients' Rights and Responsibilities, incorporating them into daily operations and improvement activities.
  • Environment Steward: Contribute to creating a safe, clean, and comfortable environment for patients and staff.
  • Confidentiality Guardian: Maintain the security and confidentiality of sensitive data, aligning with hospital policies and legal regulations.
  • Infection Control Advocate: Follow established procedures to mitigate infection risks.
  • Relationship Builder: Foster positive working relationships, encouraging a cooperative work atmosphere.
  • Communication Expert: Engage effectively and professionally with patients, physicians, and staff, providing clear and concise reports.
  • Educator: Monitor staff's ability to assess patient and family needs and ensure collaborative interdisciplinary education.
  • Community Connector: Guide staff in identifying age-appropriate resources and community support for follow-up care.
  • Talent Developer: Encourage staff retention through development opportunities and conduct caregiver evaluations according to standards.
  • Culture Shaper: Exemplify and communicate a culture of leadership, trust, learning, and teamwork.

What You'll Bring:

  • Educational Background: Bachelor's Degree in Nursing and Master's Degree in Nursing, Health Administration, Business, or related field.
  • Experience: Minimum 5+ years in a leadership care management role in a major acute care medical center. Experience managing both nursing and social work teams. Union experience strongly preferred.
  • Leadership: Over 7 years of progressive leadership in healthcare delivery.
  • Licensing & Certification: Oregon RN license upon hire; certification in Case Management within 3 years.
  • Innovation & Problem Solving: Expertise in population health strategies and implementing data-driven interventions to enhance patient experience and care quality.
  • Relationship Management: Proficiency in leading cross-functional teams and building strong professional relationships.
  • Communication Skills: Outstanding written, verbal, and listening skills.
