Provides support for individuals and/or groups of persons with disabilities in residential and community settings. Implements services and supports to address the person's goals identified through each person's Individual Support Plan. Assists the person(s) receiving services in identifying and communicating his or her requests and needs for services and supports. Relies on supervision and direction from the Community Services Coordinator to accomplish assigned tasks.Job Responsibilities:Providing opportunities for choice and encouraging clients to exercise control over themselves and their environmentHelping to identify personal outcomes that will best suit their individual interests, cultural backgrounds and dreams for the future.Work with an interdisciplinary healthcare team to identify the appropriate support methods necessary to achieve the stated goals.Implement services and supports that will assist your client to achieve personal goals and desired outcomes as identified through an Individual Support Plan.Assist them with in the implementation of personal plans, goals, and objectives; working with them to facilitate their communication in terms of needs and requestsAssisting and instructing on food preparation and house cleaningHelping clients to identify and acquire valued social roles, as well as healthy personal and social habitsContinuously assessing the functional levels for your client, motivating clients and providing consistent reinforcement, assessments, and feedback for all learning activitiesDispensing medication, monitoring dosages and making notes in charts in accordance with your level of trainingTransporting and accompanying clients to and from day programs, work sites, and community activitiesSupporting clients at vocational sites to help meet their responsibilitiesAll other duties as assignedJob Requirements:High School Diploma or G.E.D., Associate Degree in a health related area; or two years' experience in a health related fieldExperience working with individuals with developmental and intellectual disabilities preferredValid drivers' license and automotive insuranceFlexible schedule and availability to provide on-call coverage as neededProvide CPR after certificationCPR, first aid, NCI and/or MedTech certification, a plusPre-employment screening:Complete criminal backgroundName checked in the registries.
