Diabetes Clinician - Mosaic Life Care : Job Details

This position participates in the interdisciplinary team. Provides service to inpatient and outpatients with attention to diabetes management and education. This position will explore customer's diabetes management and education needs and find creative solutions to satisfy those needs. This position in St. Joseph works under the supervision of the Vice President of Medial Services and Maryville works under the supervision of the Chief Operating Officer and are employed by Mosaic Health System. Duties Identifies and utilizes available and appropriate community services to enhance Diabetes Care services based upon customer needs. Conducts patient assessments, interventions, discharge planning if indicated, outcomes and goals in the medical record in compliance with Diabetes Care and regulatory guidelines Responsible for maintaining ADA recognition and participating in the evaluation of the DSME services effectiveness.
