Dementia Care Counselor-Willing Hearts, Helpful Hands

Per Diem

Willing Hearts, Helpful Hands is an innovative community program serving family caregivers in Queens, Nassau, and Suffolk Counties by connecting them with a circle of help that includes trained volunteers who provide an array of respite services. Our mission is to improve the quality of life for family caregivers of older adult loved ones by engaging the broader community to provide assistance and respite services. Our program is growing and we are seeking a Dementia Care Counselor to play a key role as our program expands.

As a Care Counselor with the Willing Hearts, Helpful Hands team, you will be responsible for crisis intervention counseling and leading support groups, and assisting with intake and enrollment of caregivers, assessments, case management and referrals.

Position Qualifications

  • Experience working with diverse older adults and their family caregivers.
  • Minimum one year of experience in the delivery of social services, community outreach, or working in a care management/care coordination role, strongly preferred.
  • College degree in Social Work, Counseling, Health Care Administration, strongly preferred.
  • New York State Social Work license, is a plus.
  • Excellent attention to detail, with the ability to communicate pleasantly and effectively with a diverse range of people.
