Check us out online: www.nychealthandhospitals.org careers.nychhc.org Docs4nyc.org Job Description PURPOSE OF POSITION: The Critical Care Pharmacy Specialist will assist the healthcare team and physicians in pharmacotherapy, and provide pharmacokinetic consultation. The Critical care Pharmacist will work to ensure the safety and effective use of medications for critically ill and injured patients. In the absence of the pharmacy manager, a Level Ill pharmacist may be required to assume the essential responsibilities and perform the duties of the pharmacy manager. Adheres to Federal State and local laws and other regulatory requirements regarding pharmacy and institutional practice. ROLES AND RESPONSIBILITIES: Unit Specific: Ability to work independently with minimal direct supervision. Exercises sound professional judgment when managing the medication therapy of critical care patients as part of an interdisciplinary team. Participates in rounds. May rotates between areas, tours, days, and shifts. Tasks include bur are not limited to:

  • Provides pharmaceutical care in Critical Care practice areas to patients from a fundamental to advanced level.
  • Participates in rounds, provides drug reviews and monitoring, drug information, patient counseling, creating protocols and policies.
  • Supervises and/or coordinates the operation of a Pharmacy unit or major function.
  • Ability to set priorities, make critical decisions, and respond quickly to emergency requests.
  • Prepares departmental reports, including the performance of complex statistical analyses.
  • Facilitates the development, implementation and maintenance of processes to improve Pharmacy operations promote desirable patient outcomes through the use of safe, appropriate, cost-effective medication therapies
  • Minimum Qualifications For Level I: 1. Possession of a valid NY State license to practice pharmacy. For Level II: In addition to meeting the qualification requirements of the lower level: 1. Two years of post licensure experience; or 2. Completion of a post graduate specialty residency or fellowship program; or 3. Current Pharmacy Board Certification. For Level III: In addition to meeting the qualification requirements of the lower levels, an additional: 1. One year of post licensure supervisory experience or specialty clinical experience.
