Crisis Case Manager

Join our Mobile Crisis Response Team in Hays, Kansas at the Crisis Intervention Center. We are launching one of Kansas' first Crisis Intervention Centers and want you to be part of this groundbreaking journey. This center will transform mental health care across northwest Kansas, easing the burden on local hospitals and law enforcement, shortening inpatient waitlists, and providing local access to critical mental health care for those in need. This will be a 24/7 inpatient facility, so there will be multiple shifts available for a variety of positions. Now is the time to raise your hand and secure your spot!

As a Crisis Case Manager, you'll be on the front lines of mental health care, responding to individuals in urgent need right where they are, whether that's in their home, at work, or in the community. You'll be part of a dynamic Mobile Crisis Response (MCR) team working to stabilize crises before they escalate to hospitalizations or emergency room visits. Every day is different, and your impact will be immediate and meaningful.

In this role, you will:

  • Deliver mobile crisis intervention services in real-time, meeting individuals face-to-face in their moment of need.
  • Support patients within our Crisis Intervention Center by helping them reach treatment goals and avoid hospitalization when possible.
  • Coordinate with treatment teams to monitor patient progress and ensure continuity of care.
  • Provide crisis support via telehealth when in-person response is not feasible.
  • Respond to critical incidents with flexibility, including after-hours and on-call shifts.
  • Accurately complete clinical documentation in compliance with regulatory and organizational standards.
  • Undergo extensive training in trauma-informed care, crisis de-escalation, suicide risk assessment, emergency protocols, and more to ensure you're well-prepared and well-supported in the field.
  • Provide transportation to clients when needed, making mobility a key part of your service.
  • Collaborate with other programs (such as Screening & Referral or Assertive Community Treatment) to deliver wraparound support to clients.

This is more than a jobit's a mission. You'll be helping individuals regain stability and dignity during some of the most challenging moments of their lives.

At High Plains Mental Health Center, we're here for you not only for the individuals and communities we serve, but for every member of our team. Our mission drives everything we do, and that includes supporting our employees with the same compassion, dedication, and care we offer our clients. When you join us, you become part of a purpose-driven organization where your work has a real impact. You'll provide critical mental health services to those who need them most, while being part of a team that values your expertise, supports your well-being, and invests in your professional growth through continuous learning and career advancement opportunities.
