Counselor, Drug and Alcohol - LifePoint Health : Job Details

Provide service coordination (case management) for assigned consumers in the community (outside of traditional office settings.) and provide on-call crisis coverage. Conduct comprehensive assessments and provide ongoing monitoring of mental illness symptoms. Act as liaison and consult with community agencies and families to maintain coordination in the treatment process. Coordinates immediately with psychiatric inpatient service providers when indicated to ensure that lengths of stay are utilized most appropriately and take a lead role in rehabilitation services. Coordinate with outside services when detoxification is necessary and establish linkage to outpatient self-help programs and residential facilities. Provide individual and group substance use counseling. Provide ongoing assessment, problem solving, skills training, supervision, and environmental adaptations to assist clients with activities of daily living (ADLs).
