Coordinator of Care - Hospice/Home Care - Days

Under the supervision of the Patient Care Director, the Coordinator of Care is responsible for coordinating and integrating high-quality, cost-effective care delivered to patients across an episode of illness or throughout the entire continuum of health care delivery. You will be responsible for assessing the patient's/family's/caregiver's post-acute care needs, facilitating Homecare or Hospice services as necessary after hospitalization. Collaborates with the social workers/case management departments at different facilities and hospitals throughout Westchester County in the assessment of the patient/family post-discharge continuing care needs in the community.

This position requires an RN experienced and knowledgeable in Certified Home Hospice care and Certified Home Health Care. An understanding of the Medicare benefit for home health care and Hospice care, and end-of-life disease processes is essential. You'll be a frontline contact for patients, families, physicians, and hospitals on behalf of Jansen Hospice & Palliative Care and Lawrence Homecare of Westchester referrals. This is a full-time day shift opportunity Monday - Friday from 10:00am - 6:00 pm or 7:00 am - 3:00 pm. This position supports both Jansen Hospice and Palliative Care and Lawrence Homecare of Westchester a Certified Home Care Agency.

Preferred Criteria

* Two years of case management in an inpatient, HMO, insurance, or other outpatient setting

* Ability to work as a strong, committed member of our interdisciplinary team of professionals.

Required Criteria

* 3-5 years of health care experience in an acute care setting

* Current New York State RN license

* BCLS certification (American Heart Association)

* Ability to function as core member of an interdisciplinary team

* Excellent communication skills and computer proficiency required.
