Provision/coordination of services aimed at active rehabilitation and support services to achieve restoration of functioning and the achievement of community integration; maintaining own appointment schedule and meeting work productivity requirements. Other duties as assigned by management staff. Productivity requirement of 60% face to face contact weekly. Bachelor's Degree in a field other than a helping profession shall have a minimum of two years' professional experience working in a community based behavioral health setting.
