  • Community Paramedic has a role that expands the traditional scope of paramedics to provide more comprehensive healthcare services within the community as well as providing basic and advanced life support. Including medical evaluations in hopes of reducing morbidity. Will also deliver outreach education to improve the overall health of the community.
Essential Duties and Responsibilities:
  • Conducting Home Visits: Performing physical assessments, taking vital signs, patient education, preventive care, and managing chronic diseases.
  • Basic Medical Procedures: Conducting in-home lab draws, wound care, starting and managing IVs, and performing electrocardiograms.
  • Preventive Care: Engaging in preventive care and post-discharge follow-up to reduce hospital readmissions.
  • Conduct home health assessments to evaluate patients' physical condition, medication adherence, and overall safety of the living environment.
  • Perform clinical procedures such as wound care, blood draws, vaccinations, and monitoring of chronic conditions like diabetes or hypertension.
  • Provide patient education on managing chronic diseases, nutrition, medication management, and injury prevention to promote self-care and reduce hospital readmissions.
  • Coordinate care with primary healthcare providers, specialists, and other healthcare professionals to ensure a comprehensive approach to patient care.
  • Participate in community health screenings and preventive health programs to identify health issues early and provide appropriate interventions.
  • Deliver preventive care services, including vaccinations and health screenings, to reduce the incidence of common health conditions.
  • Assist with medication management, including reconciling medications and educating patients on proper medication use.
  • Respond to non-emergency calls for patients with chronic illnesses or those needing follow-up care, reducing unnecessary emergency department visits.
  • Develop and implement care plans for high-risk patients to manage their conditions effectively and prevent complications.
  • Connect patients with community resources and social services, such as food assistance, housing, and transportation services, to address the social determinants of health.
  • Provide mental health support and referrals to mental health services for patients experiencing stress, depression, or other mental health issues.
  • Reducing the per capita cost of healthcare, improving the patients experience of care, increase access to social services including substance use treatment.
  • Performs related job duties within the scope of a paramedic.
  • Observe, record and report to physician the patient conditions and reactions to treatments, drugs and significant incidents.
Education
  • High School Education or equivalent required
  • Missouri State approved EMT-P course
  • Missouri State approved Community Paramedic course
Work Experience
  • 2-year Paramedic clinical experience
Certification/License
  • Current Missouri Community Paramedic License
  • BLS, ACLS, PHTLS, PALS
  • Valid Missouri class E driver's license
Mental/Physical Requirements
  • Considerable mental concentration required. Lifting up to 50 lbs., turning activities and nearly constant walking required. Standing, walking, sitting, lifting (50 lbs), turning, carrying, pushing, pulling, stooping, crouching, twisting, reaching. Driving for extended periods of time. Dealing with traumatic and stressful situations.
Working Conditions
  • Frequent exposure to infectious disease and hostile persons at times. Frequent exposure to communicable disease via blood and other body fluids. Minimal physical discomfort.
