Position Description

Overview

The Community Outreach Specialist reports to the Director of Outreach with accountability for providing strategy, judgment, organization, and evidenced-based analysis to influence decisions, and directly to meet CINQCARE requirements. They should embody CINQCAREs core values, including, Trusted, Empathetic, Committed, Humble, Creative and Community-Minded. At CINQCARE, we dont have patients or customers we have Family Members.

Job Responsibilities

The Community Outreach Specialist (COS) will have the following responsibilities:

  • This?role is responsible for primary work in the community with specific target populations to educate, enroll, and manage CINQCAREs Family Members.
  • Convey the services of programs to underserved CINQCARE Family Members of the community, highlighting the purpose and the impact of that program.
  • The COS will be responsible for the recruitment of new CINQCARE Family Members.
  • Assist care team to help CINQCARE Family Members develop health management plans and goals.
  • Assist care team in coaching CINQCAREs Family Members on problem solving, effective management of their chronic health conditions, and self-care.
  • Assist CINQCARE s Family Members in understanding care plans and instructions, motivate patients to be active and engaged in self-management and promote treatment adherence.
  • The COS will be responsible for documenting activities, service plans, and results in an effective manner while strictly adhering to the policies and procedures in place.
  • The COS will serve as a liaison at sponsored community outreach functions.
  • The COS will assist CINQCARE Family Members in utilizing resources, including scheduling appointments, and assisting with completion of applications for programs for which they may be eligible.
  • Assist CINQCARE Family Members in accessing health related services, including but not limited to obtaining a medical home, providing instruction on appropriate use of the medical home, overcoming barriers to obtaining needed medical care and social services.
  • Assist CINQCAREs Family Members in conduction community strength and Needs Assessment as part of the care coordination process.
  • Assist our CINQCAREs Family Members in translation and interpretation where appropriate.
  • Advocate for harm reduction, promoting health, and wellness, utilizing health volt as tool for implementation.
  • Addresses all CINQCAREs Family Members concerns in a timely and efficient manner and reports outcomes to the Care Coordinator for resolution.
  • Perform other job-related duties as assigned.

Qualifications

The Community Outreach Specialist should have the following qualifications:

  • Education: High School, HSE or GED diploma; Associate degree preferred.
  • Experience:
    • Proficiency in all Microsoft Office applications, including, the use of online reference and information resources
    • At least 1-2 years of Community Outreach experience required.
    • Bilingual preferred.
    • Sales experience required.
    • At least 1 year of experience in a health and social services environment.
  • Entrepreneurial: CINQCARE seeks to fix gaps that have persisted for generations in the delivery of care to Black and Brown populations. This position is accountable for ensuring CINQCARE is positioned to innovatively deliver on its promise.
  • Communication: Excellent verbal, written communication and presentation skills; ability to clearly articulate and present concepts and models in an accessible manner to CINQCAREs team and Family Members. .
  • Relationships: Ability to build and effectively manage relationships with business leaders and external constituents.
  • Culture: Good judgement, impeccable ethics, and a strong team player; desire to succeed and grow in a fast-paced, demanding, and entrepreneurial Company. Bilingual/bicultural encouraged to apply.

Required:

  • This position will be mainly in the field a verifiable good driving record, valid drivers license, and reliable transportation is required.
  • Community-minded.
  • Community Outreach experience.

Location: Buffalo, NY

Compensation: $20-21 hrly.
