Job DetailsJob Location Hospice Savannah Inc. - SAVANNAH, GA DescriptionSchedule: Monday - FridayProgram Information The CAPABLE program - short for Community Aging in Place-Advancing Better Living for Elders - is a home-based, person-centered program designed to help older adults live independently and safely in their own homes for as long as possible. PACE Georgia is dedicated to empowering aging adults to live with dignity, independence, and quality of life by delivering compassionate, comprehensive, and coordinated care. PACE stands for the Program of All-Inclusive Care for the Elderly - a model of care that supports older adults who wish to remain in their homes and communities rather than move to a nursing facility. Through PACE Georgia, we partner with participants and their families to provide personalized healthcare, social support, and community-based services that honor the unique needs and values of every individual we serve. Position Responsibilities
  • Initiate and maintain contact with community referral sources, agencies, health care/human services professionals and other community organizations. This includes and is not limited to senior affordable housing communities, acute and post-acute care providers, faith-based organizations, senior centers, and other locations for direct to customer promotion of the CAPABLE and PACE program.
  • Responsible for identifying and building effective relationships with community referral sources, agencies, healthcare/human service professionals and other community groups.
  • Attend and participate in monthly departmental meetings, scheduled trainings, in-service training, mandatory annual in-service training, and educational classes as required/needed.
  • Promote and maintain collaborative relationships with managers, peers, and customers by effectively fostering a team environment, building consensus, and resolving conflicts.
  • Work collaboratively with Hospice Savannah Inc and parent organization to enhance brand in the community, identify sales opportunities, establish relationships with referral sources, and ensure operations and sales growth strategies are aligned.
  • Consistently meets or exceeds assigned monthly outreach metrics by executing targeted engagement strategies and tracking performance outcomes
  • Act as a liaison between client/participant and Intake Team as required to resolve problems, provide information on services, and maintain positive relations.
  • Consults with and advises CAPABLE and PACE staff members on the relationship of social, emotional, and cultural factors impacting access to health, medical care, and community resources.
  • In concert with the PACE IDT, refers clients/participants and families to appropriate community resources, acts as a liaison with such organizations, and serves as a care advocate for CAPABLE and PACE client/participants at all times.
QualificationsMINIMUM REQUIREMENTSRequired: Graduate of an accredited school with a Bachelor's degree in Business, Marketing, Education, Health Care Administration or related field. Case Management or Social Work experience preferred. Preferred: Minimum 2 years' experience in a health-related area. Proven ability to generate and maintain relationships with new and existing accounts including physicians, healthcare professionals, community agencies, patients, and families. Existing relationships with providers in the community preferred. Must be highly motivated, energetic and possess strong interpersonal relationship and communication skills. Ability to work with others as an effective team builder and team player Other Required Credentials: Must have a Driver's License. Ability to travel in community. Proven ability to effectively handle multiple priorities with exceptional organizational and time management skills. Ability to work autonomously, take initiative, set priorities, organize work, and make independent decisions. Comprehensive expert proficiency with Microsoft product suite (MS Word, Excel, Power Point, etc.); basic knowledge and experience with electronic mail and calendaring system. Knowledge of social service delivery systems necessary, experience preferred. Ability to type with speed and accuracy. Ability to use other software is required to perform the essential functions of the job. Be medically cleared for communicable diseases and have up-to-date immunizations and vaccines before engaging in client/participant care. Have not been convicted of criminal offenses involving Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act.
