Community Liaison - Home Health, Hospice & Palliative Care

Location: Springfield, Oregon

Schedule: Full Time | Day Shift | FTE: 1.0

Salary Range: $33.89 - $50.85 /hour (Dependent on experience and qualifications)

PeaceHealth is seeking a passionate Community Liaison to help expand access to compassionate care across our Home Health, Hospice, and Palliative Care services. In this dynamic role, you'll build relationships with referral sources-including hospitals, skilled nursing facilities, and assisted living communities-while educating healthcare professionals and patients about our comprehensive care offerings.

What You'll Do:

  • Identify and engage with potential referral partners within your assigned territory

  • Serve as a key connector between providers and PeaceHealth's Home and Community Care teams

  • Create and execute outreach programs to grow awareness and increase patient referrals

  • Deliver education to patients, families, and providers on PeaceHealth services

  • Collaborate with internal teams to identify growth opportunities and improve access

  • Maintain accurate records of outreach activity and referral partner engagement

What You Bring:

  • Bachelor's degree or equivalent experience

  • At least 3 years' experience building business relationships in home care, hospice, or healthcare outreach

  • Valid Driver's License and proof of auto insurance required.

  • Strong communication and presentation skills

  • Proven success in sales or marketing

  • Proficiency with Word, PowerPoint, and CRM tools

  • A collaborative, results-driven mindset with the ability to work independently
