Community Health Worker - Transportation - Aspire Indiana : Job Details

Community Health Worker's (CH - Ws) are flexible members of a health care team who work with vulnerable populations to improve health outcomes. The goal of the CHW is to improve the health of the community by increasing health knowledge and self-sufficiency through outreach, community education, informal counseling, social support, and advocacy. The Community Health Worker - Transportation will provide and/or coordinate transportation services to clients and employees and coordinate Aspire fleet maintenance. They will also maintain contact with clients by picking them up at their homes and making telephone calls, promote a medical home model where the CHW, healthcare team and other resources interact to meet the client's needs for services and education and provide referrals as needed.
