Growing together.The Community Health Worker position will be primary care manager for members with complex medical or behavioral needs who aren't otherwise managed by the team's clinicians. The Community Health Worker works assertively to locate and engage members in the community. They will develop individualized plans of care that focus on the members' definition of success. The Community Health Worker identifies and overcomes barriers to goal achievement, including SDOH barriers. This position will work in collaboration with the interdisciplinary clinical care management team (Pod) to advance interventions aligned with a member's readiness to change. The Community Health Worker will approach their member work with an understanding of how inequities drive health disparities. The Community Health Worker will promote health equity.Working Schedule: Schedule is Monday through Friday, any 8-hour between the hours of 8:00 am to 5:00 pm Eastern Standard Time.Location: Remote in the state of Florida.If you reside in the state of Florida, you will enjoy the flexibility to telecommute* as you take on some tough challenges.Potential for future minimal travel.Primary Responsibilities:Partner with clinical care management team to manage complex members in the communityLocate and engage members in the communityConduct screenings telephonically or virtually, to assess member medical, behavioral, functional, cognitive and psychosocial needs, i.e. health risk assessmentEngage members over the phone to educate about their healthReview available member service records and relevant documentation (e.g., utilization history, functional level, stratification information, current plan of care)Conduct comprehensive member assessment that includes bio-psychosocial, functional, and behavioral health needsUtilize interviewing techniques and active listening to collect and retain member information and incorporating responses as they are presented to complete assessmentIdentify when supplemental assessments are needed and conduct supplemental assessmentsIdentify member service needs related to health concernsIdentify urgent member situations and escalate to next level when necessaryEngage members to participate in the assessment process and collaboratively develop a plan of care based on their individual needs, preferences, and objectives with nursing oversightWork with members to develop healthcare goals and identify potential barriers to achieving healthcare goalsIdentify member support systems available and incorporate into plan of careReview plan benefits and identify appropriate programs and services based on heath needs and benefitsIntegrate health care and service needs into a plan or recommendation for member care and serviceWork collaboratively with the interdisciplinary care team to ensure an integrated team approachCollaborate with members to create solutions to overcome barriers to achieving healthcare goalsIdentify relevant community resources available based on member needsRefer members to appropriate programs and servicesFacilitate member choice of preferred providerProvide informal counseling and social support resourcesAdvocate for individuals and communities within the health and social service systemsModerate work experience within own functionSome work is completed without established proceduresBasic tasks are completed without review by othersSupervision/guidance is required for higher level tasksPerforms other duties as assignedWhat are the reasons to consider working for UnitedHealth Group?
