Facility:Dayton Children's - Main CampusDepartment:Community Behavioral Health ServicesSchedule:Full timeHours:40Job Details:The Community Health Workers (CHWs) serve the patients and families referred from Dayton Children's Hospital outpatient clinics, regardless of their county of residence. The Community Health Workers (CHWs) use appropriate screening and interviewing techniques to build relationships with patients and families and link them to appropriate community health and social service resources. The CHWs serve as patient advocates and provide community outreach including home/school visits. They provide education as identified by the specialized physician and medical team, especially pulmonary and complex care clinics. In addition, the CHWs provide community education through participating in various community outreach activities and events.

Department Specific Job Details:

  • full time, 40 hours

  • day shift, M - F

  • 8:00 AM - 4:30 PM

  • Some after hour days, 9:00 AM - 5:30/10:00 AM - 6:30 PM

  • no holidays, some weekends for outreach events

  • serving the Hope Center and the main campus hospital

Education Requirements:

Associates (Required)

Certification/License Requirements:

Community Health Worker (CHW) - Ohio Board of Nursing, CPR - American Red Cross
