Care Coordination Promotes positive customer relations in dealing with patients, visitors, and other employees. Interacts with clients to promote a full range of health services; determines the benefit of each health program after assessing clients' willingness to enroll in available programs. Provide cultural and linguistic health education information and current resource materials to individuals and families. Utilize a holistic approach (considering physical, psych/social, spiritual, educational, safety and related criteria) appropriate to the age of the clients served in the assigned service area. Refers families to clinical, social, and financial support using an established algorithm to assess patient and family needs. Assist patients with accessing community-based resources by supporting them with applications, transportation needs and follow up on any referrals made to ensure patient utilizes identified community based resources.
