We're looking for a CHW who is kind and professional, listens to patients and seeks solutions.About the Role:As a CHW you will assist patients and families with accessing, navigating and connecting with internal providers and services, outside community resources and programs. Responsibilities include:Assists patients in accessing community resources upon provider referral to, but not limited to, food pantries, clothing, shelter, transportation, education, parenting or other services as neededAssists patients in accessing health care by assisting in the completion of Medicaid, Child and Family Health Plus and Sliding Fee Scale applications. Connects patients to appropriate facilitated enrollersAssists patients in accessing and completing referrals as ordered by providers. Obtains prior authorizations as needed. Documents and tracks referrals until completionConducts community outreach to identify people who are experiencing homelessness and educate them about health needsSupports people who are experiencing homelessness in accessing health care services at the Health CenterServes as health and community resources coordinator for patients and providersConsults with providers regarding patient needs and making appropriate referrals or linkagesAdvocates for patients when interacting with community agencies and servicesConducts outreach and speaks to community groupsAssists in developing Health Center staff in improving patient navigation, decreasing barriers for patients in accessing care and connecting patients to care as appropriateYou will be primarily based at our Blasdell location, 4233 Lake Ave., Blasdell, NY 14219.What it's Like to Work at Neighborhood:The top three words employees say describe the work environment are: teamwork, supportive, kind. Available for extended hours once a week 7-9 p.m.Experience in Community Health, Patient Resource Coordination or equivalent experienceMedical office and medical terminology knowledge and experience preferredExceptional attention to detail and follow-throughStrong skills in utilizing Electronic Health Records (EHRs), Microsoft Office Suite, and other computer software packagesSuperb organizational skillsStrong influencing skillsSelf-starter with the ability to work proactively and take initiativeResults-oriented; creative and persistent in accomplishing tasksSolutions-driven with a systems orientationHighly responsive, timely, accurate and thorough in follow-up and follow-throughHighly flexible, positive and results-oriented attitudeExcellent in managing shifting priorities and timelinesExcellent oral and written communication skillsStrong ability to diplomatically and effectively manage interactions with individuals at all levels and job functions, both inside and outside the organizationExcellent judgment and discretion Must be available to work Monday-Friday between 8:30 a.m. and 4:30 p.m.What We Offer:Compensation: $22.25 an hour based on a full time, 40 hour workweekBenefits: You'll have options for medical, dental, life, and supplemental insurance.
