Position Purpose
In this two year grant the Community Health Worker assists homeless individuals in the local community gain access to key social service and healthcare providers/services to better their health and improve their lives.
What you will do
Improve access to health and human services for homeless population through in-person interactions, telephone contacts, and coordination of referrals
Facilitate or conduct enrollment in available federal, state and local programs to increase access to health care and support services
Maintain frequent contact with community agencies serving vulnerable homeless and at-risk of homelessness populations
Provide individual & community support. Serves as a community liaison between target population and agencies and providers
Provide transportation assistance necessary to ensure the success of the populations served
Minimum Qualifications
CHW Level I- Minimum high school diploma or equivalent
CHW Leve II- Minimum Bachelor's Degree in human services or related field
Basic Life Support (BLS) certification
Current Driver's License in good standing
BILINGUAL (SPANISH/ENGLISH) STRONGLY PREFERRED
