Community Health Worker - Healthy Neighbor Program - Virtua Health : Job Details

The Community Health Worker (CHW) will be Healthy Neighbors frontline presence in the community. The CHW will be: working to improve patient health by providing social support, advocacy, and resource navigation; conducting health and wellbeing check-ins at clinics, community sites, patient homes and remotely by virtual visits and phone calls; connecting patients with follow-up clinic appointments, social resources, reinforce diabetes and hypertension health education and collaborate with patients to identify barriers to chronic disease management and work toward solutions; building strong relationships with patients and their families. Position Responsibilities:Perform needs assessments and enroll patients in Healthy Neighbor by meeting with prospective patients in-clinic, at community sites & through remote outreach.
